ABSTRACT

ABSTRACT: Colonoscopy is a minimally invasive technique used to assess the large intestine through direct inspection of the intestinal mucosa. When associated with histopathological examination of fragments collected from the intestine, the definitive diagnosis can be obtained. This retrospective study evaluated colonoscopy and histopathological exams of the large intestine and ileum of dogs with gastrointestinal disorders admitted at the Veterinary Hospital of the Federal University of Minas Gerais (UFMG) and the Veterinary Hospital São Francisco de Assis to determine the frequency of injuries, their distribution in the intestinal segments, and the relationship of the findings observed in these two analyzes. The colonoscopy and histopathological findings of the case series were described using absolute and relative frequencies, as well as nature and intensity classification of the findings. Cohens Kappa coefficient was obtained to assess the concordance of nature and intensity classifications between colonoscopy and histopathology, and its 95% confidence interval constructed. The analyses were performed using the Software SAS University Edition. It was observed a moderate agreement between the classification of the nature of the findings by endoscopy and histopathology (Kappa coefficient = 0.39, CI = 0.20-0.59). This can also be observed when assessing the frequency of similar diagnoses between the methods, since only 39 (72.22%) were consistent, i.e., 15 (22.78%) diagnoses differed depending on the nature of the finding, which could have a great influence on the final diagnosis if histopathology was disregarded. For the intensity of the injuries, little agreement was observed between the methods (Kappa coefficient = 0.1243, C = -0.05-0.30). This was even more evident in the frequency of similar diagnoses in terms of intensity, of which 20 (37.04%) were similar and 34 (62.96%) were different. Inflammatory affections are the most frequently observed alterations in the large intestine and ileum of dogs. The most common finding that reveals inflammatory changes is the lymphoplasmacytic infiltrate. As for the proliferative and neoplastic lesions, adenomatous polyps and lymphoma were common. The most affected sites of the large intestine were the descending colon and the rectum. Findings such as edema and reddening of the mucosa were frequent by macroscopy. Although the changes observed by colonoscopy and histopathology may not be similar, these techniques are complementary, which makes biopsies mandatory for a diagnostic conclusion.


RESUMO: A colonoscopia é uma técnica pouco invasiva utilizada para avaliação do intestino grosso por meio de inspeção direta da mucosa intestinal. Quando associada ao exame histopatológico, com a coleta de fragmentos do intestino, o diagnóstico definitivo pode ser obtido. O objetivo desse estudo retrospectivo foi associar os achados de exames de colonoscopia e histopatologia do intestino grosso e íleo em 54 cães com distúrbios gastrointestinais dos Hospitais Veterinários da Universidade Federal de Minas Gerais (UFMG) e São Francisco de Assis. Na colonoscopia, as alterações mais frequentemente observadas foram edema, friabilidade e avermelhamento de mucosa. Quanto à distribuição de lesões por segmento intestinal, houve maior incidência de alterações inflamatórias, das quais foram as mais frequentes, com o infiltrado linfoplasmocitário sendo o mais comum em todos segmentos analisados (i.e. reto, cólon, ceco e íleo). O cólon ascendente e o reto foram os locais de alterações mais frequentes na colonoscopia e na histopatologia. Os pólipos hiperplásicos e o linfoma foram as lesões proliferativas de ocorrência mais comum. Houve baixa concordância entre as classificações por natureza e intensidade dos achados na colonoscopia e histopatologia. Assim, não foi possível associar alterações descritas nos exames histopatológicos quanto à natureza e intensidade das lesões utilizando a colonoscopia, o que leva à conclusão de que é essencial a realização de biópsias em todos os exames para conclusão diagnóstica.

O íleo paralítico é uma obstrução do tipo funcional, na qual o lúmen intestinal está patente, comprometendo a passagem da ingesta no intestino. Este relato objetiva descrever três casos em vacas com aptidão leiteira, atendidos na rotina hospitalar e diagnosticados com íleo paralítico. Em seus históricos, os proprietários queixavam-se que os animais apresentavam redução do apetite, timpania ruminal, diminuição da produção de leite e eliminação das fezes. Ao exame físico, alguns sinais clínicos apresentaram destaque, como apatia, desidratação, redução da motilidade ruminal e intestinal, fezes em pequena quantidade e com muco, distensão do abdômen e ao balotamento constatou-se a presença de líquido. Na análise do fluido ruminal todos os animais apresentaram o teor de cloreto elevado (>30 mEq/L), caracterizando, dessa forma, um processo obstrutivo. Diante dos achados, suspeitou-se inicialmente de um quadro de obstrução intestinal. Nos casos, a conduta adotada foi realizar uma laparotomia exploratória através do flanco direito, porém constatou-se, que não existia qualquer segmento com obstrução de natureza mecânica, que justificasse as alterações físicas e laboratoriais encontradas. Diante destes resultados, configurou-se um quadro clínico indicativo de íleo paralítico. Paralytic ileus is an obstruction of the functional type, in which the intestinal lumen is patent, compromising the passage of the intake in the intestine. The objective of this study was to describe three cases in dairy cows treated in the hospital routine diagnosed with paralytic ileus. In their histories, the owners complained that the animals presented reduced appetite, ruminal tympany, decreased milk yield and elimination of faeces. At the physical examination, some clinical signs were prominent in both, such as apathy, dehydration, reduction of ruminal and intestinal motility, faeces were present in small quantity and with mucus present, abdominal enlargement and the succession produced sloshing sounds. In the analysis of the ruminal fluid, the chloride content in both was high (>30 mEq/L), characterizing an obstructive process. In the face of the findings, a diagnosis of intestinal obstruction was initially suspected. In animals, the adopted approach was to perform an exploratory laparotomy through the right flank, but it was verified that there was no segment with mechanical obstruction that justified the physical and laboratorial alterations found. In view of these results, a clinical diagnosis indicative of paralytic ileus was established. The three animals were submitted to a post-surgical therapeutic protocol consisting of antibiotic therapy, anti-inflammatory, calcium, pro-kinetic and supportive treatment. The animals showed a favourable response to therapeutic treatment, with restoration of gastrointestinal function and other physiological parameters, and was discharged after a clinical evolution ranging from ten to twelve days.

Os animais manifestaram uma resposta favorável a conduta terapêutica, com restabelecimento da função gastrointestinal e dos demais parâmetros fisiológicos, recebendo alta após uma evolução clínica variando entre dez a doze dias.

ABSTRACT

In the Amazon region, sprouting pastures of Panicum maximum cause colic and death in horses and mules. To produce the disease two experiments were performed with growing P. maximum cv. 'Mombaça' in penned horses. In Experiment 1, at the beginning of the raining season, 10 horses were fed with recently harvested sprouting P. maximum. Two horses showed colic on the 5th and 11th day after start of the ingestion. In Experiment 2, also during the raining season, the same 10 horses used in Experiment 2 were divided into two groups of 5 horses each. Group 1 received sprouting P. maximum immediately after harvested and Group 2 received the grass 24 hours after harvested. The control group consisted of four animals which were placed in individual pens and fed with Pennisetum purpureum and commercial ration twice a day. One horse from Group 2 showed colic on day 13th after the start of feeding. The other horses of both groups showed intestinal bloat. In both experiments affected horses recovered after the end of the administration. The control group did not showed clinical signs. These results demonstrated that the disease is caused by the ingestion of sprouting P. maximum during the rainy season. The results in experiment 2 showed that P. maximum do not loss toxicity 24 hours after harvesting. Clinical signs suggest that primary alteration caused by P. maximum is intestinal fermentation followed by bloat.


Na região Amazônica, Panicum maximum em brotação causa cólica em equinos durante o período chuvoso. Com o objetivo de reproduzir experimentalmente a doença, foram realizados dois experimentos com P. maximum cv. 'Mombaça' em brotação. No Experimento 1, 10 equinos receberam o capim em brotação, ofertado no coxo. Dois animais apresentaram cólica, um no 5° dia e outro no 11° dia de experimento. No Experimento 2, também durante a época de chuvas, os mesmos 10 animais foram separados em dois grupos de 5, um grupo recebia P. maximum recém cortado e o outro recebia o capim cortado e dessecado à sombra durante 24 horas antes da administração. O grupo controle de ambos os experimentos foi constituído por quatro equinos, mantidos em baias individuais, recebendo capim Pennisetum purpureum picado e ração comercial duas vezes ao dia. No grupo que recebia o capim coletado com 24 horas de antecedência, um animal apresentou cólica no 13° dia de experimento. Os demais equinos dos grupos 1 e 2 mostraram sinais de timpanismo intestinal. Nos dois experimentos, os animais afetados recuperaram-se espontaneamente após o final da ingestão. O grupo controle não apresentou sinais clínicos. Os resultados comprovam que a doença é causada pela ingestão de pastagens de P. maximum em brotação. Os resultados do Experimento 2 demonstraram que P. maximum não perde toxicidade 24 horas após a coleta. Os sinais clínicos observados sugerem que a alteração primária causada por P. maximum é uma alteração fermentativa com produção de gás e timpanismo intestinal.
